ACC, MedAxiom Collaborate on Cardiovascular Care Transformation Initiative

Recently published foundational document and AI chapter provide a framework for health systems to integrate novel paradigms of care delivery.

April 10, 2025 — The American College of Cardiology’s (ACC’s) five-year Strategic Plan includes a major initiative focused on transforming care delivery in new areas and recognizing the importance of delivering care within the communities where patients reside.1 As part of this initiative, the ACC and MedAxiom, an ACC Company focused on cardiovascular organizational improvement solutions, are partnering on a series of care transformation resources designed to provide a framework for health systems to effectively integrate novel paradigms of care into their delivery models.Included in the series are novel operational architectures tailored to specific clinical objectives, patient populations and non-clinical determinants of health, along with key implementation strategies addressing team-based care optimization, appropriate setting of care, and creation of an artificial intelligence (AI)-enabled clinician and health system.

At the heart of the series, of which the first two areas of focus are AI and ambulatory surgery centers (ASCs), respectively, are actionable resources including self-assessment tools, best practices and customizable strategies for implementation.

“We have a unique opportunity to utilize a modern framework for best practices for cardiovascular care delivery and implementation,” said Ami B. Bhatt, MD, FACC, ACC chief innovation officer; Ginger K. Biesbrock, PA-C, DSc, MPH, FACC, MedAxiom executive vice president, Care Transformation Services; and Cathleen Biga, MSN, MACC, ACC immediate past-president. In their recent Leadership Page published in the Journal of the American College of Cardiology, they affirm, “By adopting a comprehensive framework that emphasizes clinical excellence, team-based collaboration, innovative delivery mechanisms and data-driven approaches, health systems can effectively optimize care delivery and enable improved access to equitable and efficient care.”

Read their full leadership page for an overview of the initiative on JACC.org. To access the introductory framework, download the foundational document. The AI in Health Care Tool is also now available at ACC.org/AI. Future chapters focused on ASC development and other key innovations in cardiovascular care delivery will be released over the next five years.
